In consideration of our participation in Dance Supremacy Kings & Queens 2026, we, the undersigned, hereby release and forever discharge ACTS Events Management (herein referred to as the Organizing Committee), its officers, agents, representatives, employees, and all other affiliated parties from any and all liabilities, claims, damages, or costs that may arise out of, or in any way be connected with, our participation in the competition.
We understand that this waiver includes, but is not limited to, claims based on negligence, action, or inaction of any of the above-mentioned parties. We voluntarily assume all risks of injury, loss, or damage that may result from or in connection with our participation.
We further declare that we are fully responsible for our actions and performances during the competition. We confirm that we are physically fit, capable of performing the stunts and movements incorporated in our routine, and have not been advised otherwise by a qualified medical professional.
We acknowledge the challenges and demands of the competition and willingly accept the risks involved.
Furthermore, we hereby grant our consent to the Organizing Committee to access and process the personal information we have submitted in connection with our qualification and participation in the competition, with the understanding that all such information will be kept strictly confidential and used only for official purposes related to the event.
We also grant consent to the taking and use of our photographs, video recordings, statements, and the mention of our names during or in connection with the competition. We further authorize the Organizing Committee to use, reproduce, publish, or distribute the video we submitted, without limitation, for event-related, promotional, or publicity purposes, whether in print, broadcast, or digital media.
We hereby carefully read this form and agree to abide by all rules and directions of the Organizing Committee during the competition.
